Blue Care Connection Care Management and Wellness Programs* are an integrated, holistic approach to health care management that helps our members across the spectrum of health: from wellness to chronic disease to catastrophic illness. The Blue Care Connection Disease Management Programs help those who have chronic disease by providing self-management guidance and external support services. As part of the Disease Management Programs, members with these chronic diseases receive educational materials, notices about upcoming classes, and reminder notices for exams.
