This Premier Plan includes benefits for:
- Services covered by Medicare and received from Medicare-participating providers (providers who accept Medicare assignment):
- No deductibles, copayments, coinsurance, or out-of-pocket costs
- No filing of claim forms for services received in New Mexico
- No referrals required
- Services covered by Medicare received from nonparticipating Medicare providers, and services not covered by Medicare but covered by the Premier Plan:
- $100 deductible
- 20% coinsurance
- $500 out-of-pocket maximum per calendar year.

- Wellness services
- Acupuncture and additional chiropractic services
- Massage therapy and rolfing
