Prescription Drug Benefits
Mandatory generic dispensing is effective July 1, 2006. If you prefer a brand-name drug that has a generic equivalent, or if you or your provider order a brand-name drug when a generic equivalent is available, you will pay the difference in cost between the generic and brand-name drug, in addition to the generic drug copayment.

| Brand-name drug on Drug List | 30% (Minimum $50, Maximum $90) |
| Drug not on Drug List | 40% (Minimum $90, Maximum $160) |
