Health Risk Assessment
To take the Health Risk Assessment (HRA) ††:

  1. Log in to Blue Access® for Members. If you are not currently a Blue Access for Members user, sign-up now for instant access.
  2. Click on the Personal Health Manager** logo.
  3. Once on the site, click on the Take the Health Risk Assessment link in the “Know Your Risk” box.

Weight Loss Guide **
To view the Weight Loss Guide:

  1. Log in to Blue Access® for Members. If you are not currently a Blue Access for Members user, sign-up now for instant access.
  2. Click on the Personal Health Manager logo.**
  3. Once on the Personal Health Manager site, click on Weight Loss Care Guide in the in the “My Tools” box.

For Your Health Program**
To view the For Your Health Program:

  1. Log in to Blue Access® for Members. If you are not currently a Blue Access for Members user, sign-up now for instant access.
  2. Click on the Personal Health Manager logo.**
  3. Once on the Personal Health Manager site, the For Your Health box will appear. Just click on the desired selection and begin.

 

 

 

The relationship between Blue Cross and Blue Shield of New Mexico and Personal Health Manager is that of independent contractors.

This feature is available with the Personal Health Manager, which is available to members in certain health plans we offer.

These features are available to members in certain health plans we offer. You can access obesity information and a Health Risk Assessment from the Personal Health Manager via your Blue Access® for Members. The relationship between BCBS ofNew Mexico and Personal Health Manager is that of independent contractors.

These services do not apply to HMO members. As with other medical conditions, HMO members are encouraged to coordinate their care through their Primary Care Provider.
