BlueEdge Individual HSA gives you freedom of choice, flexibility, a broad range of benefits, and the most comprehensive provider network in New Mexico. Across the country, PPO networks are linked through the BlueCard® program to make it easy to find a network doctor or hospital when traveling, living, working, or studying anywhere in the United States.
When you receive care from preferred providers (doctors, hospitals, and other providers contracted with Blue Cross and Blue Shield):
- Benefits are paid at the highest level.
- There are no claim forms to complete.
- You are not responsible for any charges above the Blue Cross and Blue Shield negotiated fee or allowable amount.
- You do not need a referral to see the doctor of your choice.
After your deductible is met and your PPO coverage begins, your only other out-of-pocket expenses are any plan-specific coinsurance payments for covered benefits, plus any charges for non-covered services.
You can choose to receive care from doctors and hospitals outside the network (nonpreferred providers), but benefits will be paid at a lower level. Please refer to your benefit booklet for more information.
Benefit Overview
A Choice of Deductibles to Fit Your Budget
BlueEdge Individual HSA gives you a choice of three annual deductibles: $1,200, $1,700, and $2,600 for individuals; and $2,400, $3,450, and $5,150 for families. A deductible is a fixed amount you are required to pay before health care benefits begin.
Unlimited Lifetime Benefit
With BlueEdge Individual HSA, there is no lifetime maximum benefit. This means you have substantial protection for today and the years ahead. Please note: Certain services have maximum annual limits.
Preventive Care Benefits
Preventive care services are covered at 100 percent when received from preferred providers (annual $400 maximum, remainder applies to deductible and coinsurance).
- Well-child and adult care, including immunizations
- Screenings such as cholesterol, breast cancer, and cervical cancer
Doctor and Hospital Benefits
- Doctor office visits
- Hospital and surgical care
- Diagnostic testing
- Emergency care benefits
Other Coverage
- Rehabilitation therapy (physical, speech, occupational therapy)
- Prescription drug benefits
- Reconstructive surgery following a mastectomy
- Case Management Services
Please note: BlueEdge Individual HSA does not provide benefits for maternity (including any pregnancy-related condition) or mental health (including substance/alcohol abuse).
