Applying for BlueDirect
To enroll in this health plan, submit an application (PDF) along with a $20 application fee**. Medical screening is required and an applicant may not be approved based on health history. BlueDirect does not provide benefits for maternity (including any pregnancy-related condition) or mental health (including substance/alcohol abuse).
