With the year coming to a close, it’s a good time to remind our members to receive any preventive screenings, follow-up care or immunizations they may need. We can provide you with data and support to help prioritize outreach efforts according to members’ needs.
Why year-end outreach matters: Closing care gaps goes beyond meeting Healthcare Effectiveness and Data Information Set (HEDIS®) measures. Receiving timely care may contribute to positive health outcomes while reducing avoidable complications.
Your office may make a meaningful impact by focusing outreach efforts on:
- Well-child visits: Ensuring children receive age-appropriate preventive care, developmental screenings and anticipatory guidance
- Immunizations: Supporting children, adolescents and adults in staying current with recommended vaccines
- Cancer screenings: Encouraging eligible members to complete breast and cervical cancer screenings, which are vital for early detection
- Diabetes management: Reminding members with diabetes to complete their HbA1c testing, retinal eye exams and kidney function testing to monitor and prevent complications
- Other preventive and chronic care services: Reviewing member panels to identify any remaining screenings, tests or follow-up visits needed before the end of the year
