Provide Records to Support Quality Care for Commercial Members

We’re collecting medical records for members of commercial plans from February through April 2026 for Healthcare Effectiveness Data and Information Set (HEDIS®) measures.

How you can help

  • We will contact you by fax, phone or email to provide details about the medical records needed and how you can return or make them available for our team.
  • Patient authorization is not required to release these records. Their collection and review is considered a component of health care operations under the Health Information Portability and Accountability Act.
  • Contact the representative listed on the letter to set up remote access to your electronic medical record.
  • Supply medical records no later than five business days after you receive the request.
  • You can request on-site appointments for medical record retrieval.
  • Medical records should be furnished at no cost to Blue Cross and Blue Shield of New Mexico. Providing the records is a contractual requirement. If you use a print vendor, it remains your responsibility to submit the medical records in a timely manner.

What data we’re seeking

We collect medical record data from 2025 for:

  • High blood pressure control
  • Diabetes care − Hemoglobin A1c control and blood pressure control
  • Prenatal and postpartum care (medical records 2024 through the end of 2025)
  • Weight assessment and counseling for nutrition and physical activity for children and adolescents

HEDIS performance measures are developed and maintained by the National Committee for Quality Assurance. HEDIS is the most widely used performance measurement tool in health care.
