Standard Option Plan Benefits

Standard (LoboCare) Network Plan benefits include:

• Access to UNMH providers and facilities, plus access to additional providers such as urgent care centers, labs, pharmacies, home health care, and durable medical equipment suppliers
• No referrals required to see a contracted specialist
• Out-of-network benefits (you meet the deductible and pay a percentage of covered charges)
• Benefit Level Exceptions In some cases, you will be able to receive services outside the Standard Network and receive a higher level of coverage than would otherwise normally be available

Prescription Drug Plan

Your coverage includes a prescription drug plan. View the drug list. If you are covered under the Consumer Driven Health Plan, you have access to preventive prescription drugs that are covered with a copay. For these medications you do not have to satisfy your deductible first. You can search the UNMH HSA preventive drug list. Coverage When Travelling

The BlueCard^® Program helps our HMO members to access emergency health care services while traveling in another Plan's service area by taking advantage of local Blue Cross and Blue Shield provider discounts.

Coverage when temporarily residing away from home: For HMO members who are away from home for at least 90 days, the Away From Home Care^® program provides nationwide access to managed care providers at a participating Blue Cross and Blue Shield HMO.

Gender Reassignment and Related Supplies and Services

This plan covers gender reassignment surgery – including related services and supplies, when prior authorization has been obtained.
