Your health plan provides a way to help reduce or waive some of the costs to manage your condition. All you need to do is enroll and stay engaged in the Chronic Conditions Condition Management program.* Part of this program includes a Pharmacy Condition Management benefit that allows a $10 reduction on your copays for maintenance drugs
