Blue Cross Medicare Advantage Dual Care Plus (HMO SNP)SM Plan

Whether you're new to Medicare or thinking about switching plans, here are some important things to consider before choosing Blue Cross Medicare Advantage Dual Care Plus (HMO SNP).

  • Be sure you are eligible for Medicaid benefits with the New Mexico Human Services Department.
  • Have a primary residence in a county where Blue Cross Medicare Advantage Dual Care Plus (HMO SNP) is offered. Eligible counties are Bernalillo, Chaves, Curry, Dona Ana, Otero, Quay, Roosevelt, Sandoval, Santa Fe, Taos, Torrance, and Valencia.
  • Depending on your level of Medicaid eligibility, you may not have any cost-sharing responsibility.
  • Review the plan benefits and built-in drug coverage in the chart.
  • If you'd like to enroll in a Medicare Advantage plan, make sure you're aware of enrollment periods. Members may have the opportunity to change health plans once per calendar quarter.
