SBC documents available via our public site are for general reference only. Members and employers should log in to their secure accounts to access official plan documents.

Small Group Metallic SBC Documents

  • For groups between 1 and 50 employees
  • "Metallic" (platinum, gold, silver or bronze) health plans
  • Fully insured health plans

Blue Balance Funded SBC Documents

  • For groups between 5 and 150 employees
  • Not "metallic" (platinum, gold, silver or bronze) health plans
  • Level-funded health plans, which are considered self-funded plans

 

Mid-Market Group SBC Documents

  • For groups between 51 and 150 employees, 
  • Not "metallic" (platinum, gold, silver or bronze) health plans
  • Fully insured health plans
